The first appointment is to establish long-term ongoing care for patients age 18 years and older. During this 60min session, we will go over your diagnoses and set up goals for the treatment. A signed release of information will be required ot gather additional information from other healthcare providers.
Follow-up visits focus on tracking response to medication changes and the initial treatment plan. During this 25min visit, we will review progress towards current goals for treatment, adjust the treatment plan, provide medication refills, and discuss timeline for next follow up visits.

C&C Psychiatry, LLC is a direct pay (self pay) practice. We do not accept any insurance, Medicaid or Medicare; however, our fee schedule is simple with no hidden costs. Patients pay out of pocket for the appointment. We provide them with a superbill (medical receipt) that they can submit to their insurance company for reimbursement. While most private insurance companies reimburse 50-80% of the out-of-network cost to the patient, it is best to check directly with your insurance company to find out what will be covered.

A follow up appointment canceled with less than 48-business-hours notice is subject to a late cancel fee of $100, this applies for no-shows as well. These fees are collected from the credit card on file immediately upon cancellation and are not reimbursed by insurance. These fees are assessed not only to show the value of your provider's time, but also to underscore the importance of keeping regular appointments. If there were emergent or extenuating circumstances which kept you from keeping your appointment we would ask that you reach out to our office to discuss what can be done regarding the fee. If you have more than two unexplained no-shows, you will be discharged from the clinic, and we may ask you to seek services elsewhere.
